Access Information

Self referrals accepted as well as referrals from primary care.
Please call, complete online form, fax or walk-in to make a referral.
Individual must have an opiate dependency and a valid Ontario Health Card. Individuals under the age of 16 may be accepted in the program with a BLUEWATER Methadone Clinic physician consult.
