Program Description

14th Street Transitional Supportive Housing in Owen Sound includes 12 apartment units and provides high-acuity transitional housing support for individuals with mental illness and/or substance use issues who are homeless or at risk of homelessness. This high-intensity supportive housing model helps individuals transition successfully into stable housing while reducing the reliance on emergency services. The program offers; case management, social recreation, peer supports, primary care telemedicine, addiction counselling and ID clinic.

Access Information

Referrals are taken directly from the Grey County By-name List (BNL). If an individual is currently experiencing homelessness and is not on the BNL, a referral should be made via Grey County.
An individual must be willing to make a commitment towards mental health and/or substance use recovery, including regular engagement with 14th Street programs and services as appropriate.
