Program Description

The Mobile Health Team consists of a family physician, nurse practitioners, registered nurses, peer support, and a social worker for those who are unhoused/unsheltered, experience complex health care concerns, and are not connected to a family doctor or a nurse practitioner. The team will meet clients in the community or at one of several walk-in clinic locations across Halton Region.
