Program Description

The Mobile Community Health Centre (MCHC) Team provides community-based primary and social care to those who are unhoused/unsheltered, experience complex health care concerns, and are not connected to a family doctor or a nurse practitioner. The MCHC team will meet clients in the community or at one of several walk-in clinic locations across Halton Region.
