Program Description

A short-term, home-based program designed to provide practical, goal-oriented support directly within the comfort of your home. This flexible service addressed specific challenges faced by individuals and families, focusing on building strengths, improving relationships, and developing actionable strategies to achieve meaningful change.

Access Information

Anyone can begin a referral (i.e., child, parent, family member, teacher, doctor, FCS worker etc.) by calling to book with the Pathway Navigator.

Please note, in certain cases of custody/access arrangements (i.e., joint or unspecified custody) consent by both biological parents, or a proof of sole custody will be required. All Phoenix Centre programs are a voluntary service.
