Centralized/Coordination Access
Centralized intake is for families and/or adolescents to discuss their concerns and to be matched with services and providers. Families and/or adolescents will then be placed on the waitlist for the appropriate program as determined in discussion with the intake worker.”
Anyone can begin a referral (i.e., child, parent, family member, teacher, doctor, FCS worker etc.) by calling to book with the Pathway Navigator.
Please note, in certain cases of custody/access arrangements (i.e., joint or unspecified custody) consent by both biological parents, or a proof of sole custody will be required. All Phoenix Centre programs are a voluntary service.
