Case Management - Substance Use

Program Description

The Supportive Housing for People with Problematic Substance Use (SHPPSU) program is a partnership initiative between CCH, Breakaway Addiction Services, and The Neighbourhood Group, now TNG-Toronto Community Action Team (TCAT), which aims to help individuals with a history of problematic substance use and housing instability to live independently with support. The program uses a harm reduction approach: our program members are not required to abstain from substance use and are not required to engage in formal substance use treatment to be eligible for housing. The SHPPSU program provides 74 subsidized housing units (independent living in one-bedroom or bachelor apartments) throughout Toronto. CCH's case managers offer housing support, while the partnering agencies provide case management around daily life skills.

Referrals are through The Access Point. Please call or fill out the application form online.
Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.
