Program Description

Youth Outreach Team (YOT) offers services predominantly for youth who are:

- living with mental health and/or substance use concerns;
- open to youth who are unhoused / have no stable housing
- require frequent support;
- have not been successful in other programs;
- have frequent emergency room visits and/or inpatient stays

The YOT is a collaborative, multi-disciplinary, and intensive community-based program consisting of a Navigation and Resource Specialist, Psychotherapists, Youth Support Specialists, Peer Support Workers and Psychiatrist

Access Information

Referrals are accepted from self, families, primary care providers, and other community organizations.

Service Providers, Community Agency and Organization Referral Process:
- assess client eligibility based on the established criteria of the Youth Outreach Team
- complete the referral form with as many details about the client's needs and challenges.
- complete the HEADS-ED for the client
- email the referral form to the Youth Outreach Team Intake Co-ordinator
