Program Description

A low barrier treatment-oriented space that aims to support individuals experiencing homelessness, mental health and addiction to recover, improve their health and wellbeing and secure long-term housing.

The London HART Hub will support its clients through access to high quality evidence informed care that is trauma-informed, culturally appropriate and compassionate in nature. This referral treatment-based program connects individuals with a continuum of care tailored to their individual needs. This will be achieved by providing access to 33 transitional supportive housing units staffed 24/7 where individuals can access a safe place to rest, obtain basic needs, health care, mental health and addiction services, and many other services within one program. These accompanying supports services ensure basic needs are met while the individual works towards recovery, wellness, and housing.

Referrals are accepted from all sources, including self-referrals.
Referrals can be made by calling the Intake Team. Please call to discuss the services that are available, eligibility and space availability.

Inclusion Criteria:
- Individuals experiencing or at risk of homelessness (lacking stable, safe, permanent, and appropriate housing)
- Individuals experiencing addiction and/or mental health challenges and looking for abstinence and recovery-oriented supports
- Individuals at least 16 years of age
- Individuals who are committed to abstain from substance use while in London HART Hub Program
- Individuals who are able to adhere to HART Hub rules and code of conduct to ensure the safety and success of yourself and others
- Individuals who are committed to engage in treatment planning, and daily recovery/wellness-based programs to support in your recovery
- Individuals who are able to complete safe transfers (for toileting needs, and from/to bed) and committed to learn and develop activities of daily living with minimal support
