Program Description

The Case Manager role works within Recovery Care clinics to support clients in achieving personal goals using a wraparound, recovery-oriented model of care.
Case Managers support individuals from marginalized communities by promoting mental health, housing stability, financial security, and access to health and social services through system navigation, counselling, referrals, and practical supports. They work in close partnership with community organizations to provide trauma-informed, harm-reduction, and recovery-oriented care that fosters self-efficacy, stability, and community integration

Access Information

Individuals must be existing clients of Recovery Care to receive case management services.
