Support Within Housing - Mental Health

Program Description

Provides quality supportive housing to single individuals living in Toronto area living with severe mental illness. Residents must have formal diagnosis of serious mental illness, be on social assistance, manage own medications. They must be able to live somewhat independently or have the desire to live in a "community model" and supportive housing.

Access Information

Referrals are through The Access Point. Please call or fill out the application form online.
Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.
