Access Information

Anyone can refer, but an individual must be connected to primary care/medical support. Call or visit the website for referral.

Eligibility criteria:
- 13 years of age and older
- Medically stable with a Body Mass Index (BMI) of 16.5 or higher
- Have a Primary Care Provider (Physician or Nurse Practitioner) who can provide metabolic monitoring
- Resident of Perth County
