Program Description

Intensive case management services for individuals who are living with serious mental health related needs and would benefit from intensive supports.

Access Information

Referrals are accepted from anyone, including self-referrals.
Referrals are through The Access Point. Please call or fill out the application form online.
Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.

Individuals may also call the program directly for information.
