Outpatient Clinic - Mental Health
The Adult Outpatient Services provides multidisciplinary, outpatient assessment, treatment and consultation services.
These services include:
- Rapid Response Team (urgent support for one to three visits)
- short-term psychiatric follow-up service (medication monitoring and transition to primary care)
- psychiatric consultation (non-urgent consultation as requested by primary care)
Access to the Adult Outpatient Services is through Emergency Crisis services at Grand River hospital, the adult inpatient mental health unit (on discharge) or through Centralized Intake in the Adult Outpatient services.
Centralized Intake provides urgent access as an alternative to an emergency department visit.
Referrals are accepted from Primary care physicians; individuals are triaged and referred to the most appropriate service based on need.
