Program Description

The Community Outreach and Treatment Team (COTT) provides treatment, rehabilitation and support through an intensive service coordination model to persons living with severe and persistent mental illnesses.

Some services include outreach, injections and psychiatry.

COTT also facilitates a support group for family and loved ones of individuals living with mental illness. Education, coordination, consultation and supports regarding Community Treatment Orders are also provided through COTT.

Access Information

Referrals area accepted from Inpatient units, Emergency Departments, Psychiatrists.

Access to a psychiatrist as required. This program is for individuals who require case management or assistance with a complicated medication regime.
