Program Description

Segue Clinic is a network of addiction treatment clinics throughout the Niagara Region, Hamilton and surrounding areas, providing specialized medical treatment of opioid use disorder, providing rapid access to treatment.
Treatment modalities include Methadone, Suboxone, Kadian and Sublocade.
The program provides integrated care and works closely with community partners to provide high quality confidential service to those seeking recovery.

Access Information

Self referrals are accepted. As well as referrals from family, friends, physicians, community agencies.
Patients are welcome to walk-in at their convenience during clinic hours and receive a same day intake assessment.
If a physician or community agency wishes to refer a patient, they can simply either call the application clinic location or fax a note over providing the patient's contact information.
