Assistance to people experiencing complex and overlapping issues with rapid, low-barrier support to address substance use, mental health and homelessness challenges. Service delivery through a collaborative network of clinical, social service and care providers working together to connect people with the supports they need

- service information and navigation
- counselling
- basic needs support (food, hygiene products)
- nursing care
- transitional and supportive housing
- employment support
- peer support
- foot and wound care
- dietary support
- mobile clinic services
