Program Description

Designed to support highly marginalized populations, including those at risk of, or who are experiencing homelessness, through access to a suite of integrated, locally tailored health and human services, including, but not limited to, mental health and addictions services, primary care and supportive housing, in conjunction with case management services, to navigate additional social and employment services.

Drop-in sessions are available on Fridays.
