Program Description

The HART Hub provides integrated health and social supports for people experiencing homelessness, mental health, substance use, and other complex challenges in Downtown East Toronto.

Co-led by Regent Park CHC and Fred Victor, and supported by the Downtown East Toronto OHT, the Hub connects clients with outreach workers, case managers, and other health care providers.
