Program Description

With wraparound care at its centre, the Transitional Housing program empowers previously unhoused individuals to work towards long-term housing in the community in a safe, shared residence with 24/7 staff support.

Access Information

Eligibility Criteria:
- are chronically homeless;
- are registered on the by-name data;
- are willing and motivated to participate in programming requirements;
- and are able to take care of their own activities of daily living (hygiene, etc.) independently;
- have the capacity to live independently after this period of recovery.

Referrals for this program are made through Wellington County’s Housing Stability Department’s Housing Stability Working Group.
