Assertive Community Treatment Teams (ACTT)

Program Description

Provision of community support, rehabilitation and treatment for individuals with a serious mental illness.

Referrals accepted from all sources. Referrals are made though The Access Point. Please call or complete the online referral form.

Eligibility criteria:
Individuals with severe and persistent mental illnesses (according to the DSM-V) that seriously impair functioning in community living, with priority given to clients diagnosed with:
- Schizophrenia
- Bipolar disorder
- Schizoaffective disorder

Individuals with significant functional impairments

Clients with two or more of the following issues:
- High use of hospital services
- Persistent and/or recurrent severe major symptoms
- Coexisting substance use disorder (of more than 6 months)
- Criminal justice involvement
- Residence in an inpatient bed, but clinically assessed to be able to live in a more independent setting with the provision of intensive services
- Inability to participate in traditional office-based services
