Specialized geriatric psychiatry services through assessment, treatment, and management of risk behaviours associated with a diagnosed Major Neurocognitive Disorder for patients diagnosed with dementia with age-associated behavioural difficulties.
A completed referral form, which requests information summarizing behaviours of concern, resources used to stabilize and manage risk behaviours, client's current active and chronic medical illnesses, and the documentation of their most recent physical examination. Recent relevant laboratory/imaging results (no older than seven days); the specific lab tests are noted on the referral form. A note/verbal confirmation from the client's medical practitioner that he/she can be cared for in a non-acute setting. The physician's examination should have occurred within the past seven days with the following exceptions: a) the receiving medical physician/psychiatrist agrees to accept the patient, or b) a recent medical condition will warrant an immediate medical assessment (e.g., recent self-harm injury or medical concern). Clients with complex medical acute illnesses should have documentation within the past seven days from a medical practitioner demonstrating stability of his/her medical condition her/his medical clearance. Intake is done through the program social worker.
