Case Management - Mental Health
Provides intensive supports while working alongside clients (adults 18+) that have experienced chronic homelessness and who have mental health challenges/addictions. Supports clients in stabilizing housing, improving their emotional mental health, and integrating into the community.
Program also includes Meals that Heal, which provides home style cooked meals to those individuals who are at risk of becoming homeless. Meals are prepared fresh on site and delivered by case managers. Weekly Case Managers provide outreach and intervention to individuals who are homeless or at risk of becoming homeless. Case managers offer mental health support, coordinate medical appointments, assist with substance use and addiction issues, and connect individuals to community resources. As part of Broadview Village, Meals that Heal promotes diversity, inclusion, and equity for individuals and recognizes the importance of nutrition, safe food handling training, menu planning and meal preparation
Referrals accepted from anyone, including self-referrals.

Inclusion criteria:
- have a mental health diagnosis provided by a medical practitioner or a clinical psychologist.
- are 18 years of age or older
