Case Management - Mental Health

Program Description

Intensive case management services for adults with serious mental illness who require support to live and work in the community. Case managers assist individuals in meeting their basic needs (e.g., food, shelter, finances) and encourage the development of skills needed to live independently and improve quality of life.
