Assertive Community Treatment Teams (ACTT)
Provides service to individuals 16 years of age and over, with a history of high-level service needs caused by serious and persistent mental illness. Most of the clients have a primary diagnosis of a psychotic illness.
Provides long term, community-based, and holistic care to clients with a focus on improving quality of life and facilitating community reintegration. The team also provides support and education to families.
Referrals are accepted from self, family, physician, or psychiatrist.
Referrals are accepted by calling.
Referrals are also accepted through The Access Point.
