Assertive Community Treatment Teams (ACTT)

Program Description

Provides service to individuals with a history of high-level service needs caused by serious and persistent mental illness. Most of the clients have a primary diagnosis of a psychotic illness.

Provides long-term, community-based, and holistic care to clients with a focus on improving quality of life and facilitating community reintegration. The team also provides support and education to families.

Access Information

Referrals are accepted from self, family, physician, or psychiatrist.
Referrals are accepted by calling.
Referrals are also accepted through The Access Point.
