Program Description

This program coordinates mental health services for individuals living in South-eastern Ontario who meet the criteria for a community treatment order application: diagnosis of serious mental disorder; pattern of admission to a psychiatric facility; a condition that stabilizes with inpatient treatment; a history of stopping treatment after discharge to the community and requiring readmission to hospital.

Access Information

Referrals are accepted from family physicians, psychiatrists, and community mental health agencies. When a community mental health worker refers, the referral must be endorsed by the family/treating physician. If you are not clear where to direct a referral, call any of the services and the intake clinician will assist you.
