Program Description

Assertive Community Treatment Teams provide recovery-oriented treatment, rehabilitation and support services for those who experience a serious mental illness, who require a higher level of support in order to maintain success in community living and who have not benefited from traditional community mental health supports.

Access Information

Referrals are made from care providers and family members, however, individuals are able to self-refer. Fill out the referral form from website or call.
There is no restriction to access the program, however the standards set by the Ministry of Health state that for admission there are specific criteria for service. Priority is given to persons with a diagnosis of schizophrenia, bipolar disorder, and other psychotic disorders that have significant functional challenges.
