Program Description

The Horizon Program for Geriatric Psychiatry is a specialty care inpatient program.

We provide psychiatric assessment and treatment to persons aged 65+ with a mental health need and/or a neurocognitive disorder. We accept persons younger than 65 if a neurocognitive/functional disorder is present. We offer a continuum of inter professional services, including: consultation; assessment; treatment/stabilization; transition; and discharge. We encourage and consider the involvement and collaboration of family, caregivers and physicians as essential in providing a supportive network for our patients.

Access Information

Referrals accepted from physicians, hospitals, and long-term care homes.
Referrals are received by the Central Intake Office and reviewed by a team of clinicians. Contact the Central Intake Office or see website for referral forms.
