Support Within Housing - Mental Health
The Transition House is a social, rehabilitation home, providing 24-hour supportive care to adults primarily diagnosed with a severe mental illness.
The program assists the resident to achieve an optimal level of functioning and promotes returning to community living.
All potential admissions are screened by a multi-disciplinary selection committee.
Physical health, mental health, and substance use problems must be treated and stabilized prior to admission. Individuals must be willing to pay for rent and food. Individuals must have appropriate local medical and psychiatric support.
On-site visits are required prior to the selection committee meeting.
