Assertive Community Treatment Teams (ACTT)

Program Description

Provides a community-based combination of – treatment / symptom assessment and support-medication management, intensive case management and individual supportive therapy. The team also provides crisis assessment and intervention, rehabilitation/support, services-vocational, ADL, social, creative outreach and engagement in the Therapeutic Process by an interdisciplinary Mental Health Team.

Access Information

Referrals are accepted from a medical doctor, healthcare professional, or by self-referral.
Referrals are though Ontario Shores Central Intake.

Inclusion Criteria:
- Individuals must be diagnosed with a major mental illness and need an average of 3 intensive visits a week community-based support.

Exclusion Criteria:
- Primary diagnosis of Personality Disorder
- Substance abuse
- Developmental disability
- Disorders that can be serviced by other specialties
- Risk of being violent
- Resident of a Long Term Care Facility
