Program Description

The Shared Care clinical outreach service works with men and women who are homeless and experiencing serious and persistent mental health concerns. Shared Care teams include: full-time registered nurse, case worker, visiting family physicians and visiting psychiatrists as consultants.

Open referral. Referrals are through Access CAMH.
