Case Management - Substance Use

Program Description

System case management is provided in collaboration with other service providers for individuals considering or receiving opioid substitution therapy. Referral and discharge planning is provided. Ongoing assessments are made, and individuals' treatment plans are adjusted as required. Services are tailored to individual needs.

Access Information

Self-referrals and referrals from clinicians accepted. Call or email for referral or walk in. For healthcare providers who wish to refer patients, please call or fax us a copy of the SHC Referral Form.
