Program Description

A psychosocial rehabilitation based program which provides assessments, treatment and reintegration (discharge planning) into one’s community of choice.

Services include pharmacotherapy, group therapy and individual 1:1 support. Focus on patient centered care that encourages involvement from family, caregivers and community mental health services by developing an individualized recovery plan of care.

Access Information

Referrals for regional programs are received by the Central Intake Office and reviewed by a team of clinicians. Referrals may be made from acute care mental health services, community mental health organizations and other tertiary mental health services. Referring agents will be sent a referral package and will be contacted for further information.

Requirements:
- able to participate in recovery program at a functional level
- severe and persistent mental illness, namely complex mood and psychotic disorders
- may have co-morbid complex treatment needs (history of violence, substance use, eating disorder, personality disorder, severe deficits in functionality)
