Program Description

The Apartment program is affordable transitional housing that provides opportunities for individuals with a longstanding serious mental illness and/or substance use involvement to stabilize and establish connections with the community. The program uses a holistic, strengths-based approach to support individuals towards living a full, independent, and satisfying life. This is a time limited service that is focused on supporting individuals to live as independently as possible.

Access Information

Self-referrals accepted and can be made by calling our Central Intake Team. This program is for individuals who want to make a commitment towards their mental health and/or substance use recovery, and who are willing to engage regularly with the Outreach Support Workers (this is a requirement).
