        Community Mental Health Service Coordinators provide assistance to physicians and inpatient hospital teams to coordinate the process and development of Community Treatment Orders (CTO) for individuals being considered for a CTO.  Coordinators ensure the appropriate forms are completed, liaise with rights advisors, and facilitate the development of a comprehensive community treatment plan based on an individual's treatment and support needs once discharged from an inpatient setting. This may include making referrals for ongoing support services as part of the community treatment plan.
        
        Physicians can contact the Coordinator assigned to specific hospitals to initiate the CTO process. Community Mental Health Service Coordinators  are located in mental health units in various Toronto hospitals and provide consultation to all parties involved to assist in the various aspects involved in the development of a CTO.
