Case Management - Mental Health
Case Management services works one-on-one with a person who has serious mental health challenges and/or substance use challenges.
Workers provide on-going support as needed by the person: to establish and maintain stability in their life, reaching towards their goals, and to improve the quality of their life. Case managers assist by making referrals, coordinating and navigating health care, criminal justice, housing and social services systems beyond PARC.
Access to the program must be supported by the following admission criteria:
- History, presence and severity of a major mental illness
- History, presence or risk of homelessness
- History, presence or severity of substance use in combination with a major mental illness
- A serious constellation of primary health care issues in combination with a major mental illness.
Referrals are accepted from anyone, including self-referrals.
Referrals are through The Access Point. Please call or fill out the application form online.
Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.
