Assertive Community Treatment Teams (ACTT)
Serves adults experiencing severe, persistent, and complex mental health problems, including: those with marked impairment in social, occupational, and daily functioning; and those with special needs such as high demand for services, problematic substance use, homelessness, or involvement with the legal system.
The program focuses on Chinese, Vietnamese, Korean and Tamil populations. ACTT offers an array of culturally and linguistically appropriate mental health services.
Referrals are made by hospitals, physicians, or community organizations.
Referrals can be made through The Access Point. Please call or fill out the application form online.
Once the application is received, a service navigator will call the individual to set up an appointment to do an intake assessment.
