Provides support to racialized individuals to transition from persistent homelessness to living independently within a community. Provide an Intensive Case Management (ICM) service for people from racialized communities living with mental health or addictions and homelessness based on the Housing First model of service.

Case management services are provided to racialized individuals with mental health or addictions who require support to live and work in the community. Case managers provide ongoing support as needed by the client in order for them to stabilize, achieve their goals, and improve the quality of his/her/their life. Case managers also work to coordinate the services the client requires from across the mental health system, as well as other service systems (i.e., criminal justice, developmental, and addiction services).
Inclusion criteria:
- Individuals 15 years of age and older who have been diagnosed with mental health or addictions
- Individuals who are living in the Greater Toronto area

Provides equitable and holistic mental health and addictions services and supports for racialized communities within frameworks of anti-racism, addressing anti-blackness and anti-oppression. This program recognizes the urgent need for mental health support services and services for those who are 2SLGBTQ++ and who identify as QTBIPOC (Queer & Trans Black, Indigenous, and Person of Colour).
Across Boundaries provides community-based and in-house programs that integrate individual support, support groups, alternative and complementary therapies, skills building, social and recreational activities. Our staff practice harm reduction, sex-positivity, queer positivity, trans-inclusivity, and compassionate and trauma-informed counselling. Our services are client-centered and judgement-free.

Our Peer-led programs and services are centred around a compassionate harm reduction approach. Our goal is to prevent the negative consequences of substance use and to improve health. We provide a non-judgmental and accepting atmosphere that empowers clients instead of stigmatizing them based on their addictions.

Social rehabilitation and recreation program that integrate skills building, digital programming, support groups, alternative and complementary therapies, art therapy, creative expressions, community kitchen, individual support, counselling, and community partnerships.
Program offers a community kitchen which provides breakfast and lunch Monday to Saturday for approximately 40 clients every day. Three to four clients are assigned to work together to prepare lunch.
