Streamlined Access provides access to mental health and addictions services in York Region and South Simcoe, including Case Management; Assertive Community Treatment Teams (ACTT) and Psychogeriatric Community Treatment Team (PACTT); Regional OHT collaboration Transitional Youth Navigation; Eastern York Region OHT Primary Care Physician referral; single session counselling and navigation pathway and Supports within Housing. Peer support and Family Caregiver Navigator is also available.

A Mobile Crisis Response Team is available to provide on-scene mental health assessment, consultation and support to individuals in partnership with York Regional Police, York Region Paramedic Services, South Simcoe Police and Nottawasaga OPP.

This service available to all residents of York Region, North York and South Simcoe 24 hours per day, seven days per week, who are experiencing a mental health crisis and their families or friends. Individuals are connected to a crisis response worker who can help them through their crisis. A crisis worker can provide an immediate response in a variety of critical mental health situations including: depression, intense feelings of distress, loneliness, anxiety or fear, thoughts of suicide, feelings of rage, hopelessness or despair.

Case Management service supporting individuals to develop a plan that helps them to access community resources, reach their goals and strengthen their social and family supports. Services include: intensive case management, flexible supports, dual diagnosis, trauma services, community treatment orders, hoarding support, criminal justice services, service resolution and hospice.
Inclusion criteria:
- 16 years of age or older
- live in York Region or South Simcoe
- have a diagnosable mental health condition and/or addictions issue and/or dual diagnosis

Specialized case management provides a point of access for individuals:
- with a developmental disability and mental health needs and who have complex social, emotional, developmental, behavioural and/or psychiatric needs
- who require access to community based developmental / mental health services
- who require coordinated and creative service plan to address their immediate need

Provide a safe and supportive environment and assistance in stabilizing individuals in crisis and provides help accessing needed resources.
