West Park's Seniors Mental Health Service (SMHS) helps older adults (60 years and older) with mental health issues. We provide a comprehensive mental health assessment and short term counselling in the client's home. Reasons for referral are often due to concerns about memory loss, mood, anxiety, stress, loss or a previously diagnosed mental health disorder. We are trained to do cognitive testing. We encourage the active participation of clients, families and caregivers in this process. Our findings are reviewed by our consulting psychiatrist and we will coordinate a care plan with the family doctor with the client's consent, which includes a comprehensive report. SMHS provides practical advice and information about management, coping, resources as well as referrals to other community services for ongoing management. Telephone referrals accepted. We accept referrals from clients, doctors, community agencies and families. Our catchment includes Etobicoke, York and North York west of Dufferin. Our boundaries are roughly (East/West) from the 427 east to Jane, and (North/South) from Steeles to the Lakeshore. We also cover (East/West) Jane to Dufferin, and (North/South) Steeles to St. Clair.
