The Telemedicine Service provides non-emergency community-based service to individuals who may be experiencing mental health and/or addiction issues, including older adults with complex needs. Services include: mental health assessment, recommendations for mental health/addictions treatment, referral to appropriate community resources, and access by physicians to the Ontario Telemedicine Network for client assessment and support related to other health care needs (e.g. surgical, orthopedics).

The Addiction Mobile Outreach Team (AMOT) provides support to individuals living with substance use concerns, problem gambling or concurrent disorders. The team engages people who experience multiple barriers to accessing social and healthcare services and provides support to connect them with addiction support and services.

For individuals seeking support with mental health concerns or challenges, counselling is provided by a team of therapists on a bi-weekly basis. This goal-oriented approach helps individuals identify priorities, build coping strategies and work toward meaningful change.

Provides non-emergency assessment, consultation and treatment, with care provided by Registered Nurses in collaboration with a geriatric psychiatrist or geriatrician. Services may include cognitive testing, supportive counselling, caregiver education and planning for ongoing care.

The Addiction Program provides community-based, client-focused services for alcohol, drug, and gambling concerns for people of all ages, including but not limited to youth, families, partners, women who are pregnant and / or parenting children 0-6 years of age, and support to secondary schools in Haldimand and Norfolk Counties. Services include prevention and awareness education, assessment, individual counselling, groups, referrals to other community agencies and resources, referrals to day or residential treatment programs.

Community-based services for individuals with gambling or behavioural concerns include: prevention and awareness education, assessment, individual counselling, groups, referrals to other community agencies and resources, and referrals to day or residential treatment programs. Psycho-educational sessions available to families.

Community-based services for women who are pregnant and/or parenting a child or children zero to six years old. Assessment, community treatment, and case management are offered with strong information and advocacy components, as well as public education and community awareness.
