Support House responds to the needs of people 16+ in Halton, in need of housing stability and experiencing mental health and/or substance use concerns. We offer a full range of supportive housing options, primary care, support coordination, peer support, harm reduction trauma-informed care, navigation, and distress/crisis support. Supportive housing options range from highly intensive staff-supported residences to community supports through rent subsidies with private landlords. Our service approach for wrap-around supports for individuals includes specialties in FASD, Youth, Justice, Harm Reduction, housing, and Peer Support. The Mobile Health Team supports people of all ages and consists of a family physician, registered nurses, nurse practitioners, peer support, and a social worker. The Centre for Innovation in Peer Support provides wellness-based, peer-led self-help and social connections programming to community members, and support to organizations that have peer staff, through training in peer-support program implementation, capacity-building, evaluation, research, knowledge brokerage, and quality improvement.
