A free Healthy Lifestyle Program for individuals living with mental illness or requiring mental health support

The Outreach Team provides one-time use supplies for safer substance use to individuals across Lambton County. This includes safer injection, inhalation, snorting and sex supplies. We also provide naloxone opioid overdose reversal kits and training, sharps collection, and connections to community services. Our program uses a client-centered, evidence-based harm reduction approach that supports people to reduce substance use–related harms in ways that align with their own goals, while providing respectful, non-judgmental care. This service is anonymous. Substance use health services are fully integrated within the outreach team alongside hepatitis C care, allowing clients to access coordinated support across both areas through a single team.

The Outreach Team provides low-barrier hepatitis C testing, diagnosis, treatment, education, and support services across Lambton County. Services include point-of-care and dried blood spot testing, nursing support throughout treatment, symptom management, psychosocial support, advocacy, referrals, and system navigation. The team works to provide accessible, stigma-free care and meet individuals wherever they are on their journey. Hepatitis C services are fully integrated within the outreach team alongside substance use health services, allowing clients to access coordinated support across both areas through a single team.
