A collaborative community-based program offered to residents living at 4175 and 4205 Lawrence Ave E. Scarborough Centre for Healthy Communities has partnered with Fred Victor, Cota and Toronto Community Housing to support the community with the following team: Community Outreach Team, Case Manager (COTA), Case Manager (Fred Victor), Housing Coordinator, Peer Support Worker, Independent Support Worker, Registered Nurse and CSS Intake and Program Support Assistant. Harm reduction supplies and access to primary care are also available onsite.
