Family Peer Support Services is comprised of members caring for a loved one with a Mental Health and/or Addiction concern.
We help facilitate communication:
- among families
- mental health care givers and the community
- support members' loved ones during treatment, recovery and return to their life in the community
Combining education, advocacy, information sharing, and peer support, we encourage growth and healing in a safe and confidential place.
In collaboration with our local and regional partners, we provide opportunities for personal development and ensure our members' voices are heard by those who influence change.
Focus: Mental Health
This program provides case management services to people with serious and persistent mental illness. Clients are supported to live independently in the community through assistance with activities of daily living, advocacy, medication management and referrals to community resources.
The program helps clients to manage the symptoms of their illness through a model of psychosocial rehabilitation.

This program is designed for individuals who have a history of non-compliance with mental health care initiatives or who continue to struggle with mental health issues. The Community Treatment Order program provides education, promotion, implementation, and ongoing management of community treatment orders so that the individual, psychiatrists, physicians, and community partners can identify needs and understand available services. The CTO Case Manager ensures CTO compliance while working with the CTO team and/or Substitute Decision Maker to support clients remaining in the community and to avoid rehospitalization.

The Brief Intervention Case Manager provides short-term services to meet the immediate needs of clients and facilitates referrals, when necessary, to longer-term mental health services as required.

Court supports for people in conflict with the law, with mental illness and substance use issues. For minor offences, when appropriate mental health diversion agreements are sought to provide a voluntary alternative to a judicial ruling. The program includes short-term case management, coordination, and referral to required services, education for clients and their families, court system navigation, and screening for diversion purposes.

Community-based case management for individuals with serious mental illness and substance use issues who are in conflict with the law for minor offences.
Program includes:
- ongoing assessments
- treatment planning
- coordination of required services
- individual, group and family counselling
- conflict resolution and crisis prevention
- housing supports
- discharge planning including referral to community resources
- court diversion order coordination

Provides voluntary trusteeship to clients with serious mental illness and substance use issues whose financial situation is impacting their mental health and recovery.
Clients must be connected to long-term case management services to be eligible for trusteeship

In partnership with North Bay Regional Health Center, the program provides psychiatric consultation, treatment and referral services for adults diagnosed with developmental and psychiatric disabilities.

Community-based case management for people with serious mental illness and substance use issues who are being released from jail. The program includes coordination of required services, discharge planning and referral to community resources before their release.

Community and hospital-based case management for people with serious mental illness and substance use issues who have been found not criminally responsible and are on Ontario Review Board Orders. TCM works in partnership with the Forensic Outreach Rehabilitation Team (FORT) to provide discharge planning and case management services to individuals transitioning from the Forensic Program into the community.

Peer Connections - North Bay provides a drop-in location that is supportive, judgment-free, and a safe space for individuals struggling with mental health or substance use concerns. Staffed with experienced Peer Support Workers, they offer support ranging from community navigation, mental health/emotional support, life skills, and mentorship. The introduction of virtual and hybrid groups allows for great access to psychosocial rehabilitation groups and programs such as wellness support, grief support and various programs aimed at reducing social isolation.
