This six-bed residential service will provide a safe, supportive and supervised residential setting for individuals requiring stabilization support. Individuals may choose to stay in the crisis/stabilization residence when they are experiencing a crisis as an alternative to seeking hospitalization or as a planned admission. As this is a non-medical crisis/stabilization residential service, individuals suitable to stay must not need the services of a hospital, be able to keep themselves safe from harm, utilize the supports offered, and respect the needs of others who are staying.

All individuals living in one of Thresholds Homes and Supports housing options receive support services. Housing options range from group living, with 24 hour/seven day a week support, to shared living where support levels vary depending on the individual's need. Housing is available in various locations throughout Waterloo Region, Wellington, and Dufferin Counties. The model of service is recovery and person-centered. Intake is continuous. Wait times vary but can be lengthy, depending on availability.

This program provides long-term case management and supportive counselling to individuals with mental health issues.

The Community Homes for Opportunity (CHO) program works collaboratively with the private homeowners and their staff to provide housing, meals and support services in a home setting for people living with serious mental illness who may require 24 hour supportive care. The program is intended to support people to maintain safe housing and to access supports both in the CHO home and in the community. There is a distinct focus on recovery-oriented services that are flexible and reflect the current needs of people in the homes.

Flexible Assertive Treatment Teams (FACTT) provides a flexible, recovery-oriented model of care for individuals with complex mental health and addiction needs. Care and treatment can be “flexed” based on the needs of the individual. Focused on: - personal goal and recovery planning - supportive and therapeutic counselling - crisis intervention and crisis management - service coordination and health system navigation - assist in acquiring and maintaining housing - guidance in financial assistance and money management - health monitoring via FACT team nurses (i.e. injections, diabetes, medication delivery) - life skills and relationship building - psychiatric care

The Extraordinary Needs Program (ENP) facilitates the transition of individuals with complex mental health and other needs from hospital to an appropriate living arrangement in the community, with individualized supports reducing over time as the person stabilizes. This program provides flexible funding to meet individualized needs according to their coordinated recovery plan.

Subsidized housing is available for people who have a severe and persistent mental health issue. Housing options range from shared accommodation to individual units. Varying levels of support are provided to meet the different needs of individuals.

Provides community-based care for individuals who have severe and persistent mental health issues. The team provides: - individualized treatment - daily medication deliveries - intensive contacts with collateral services - supports in meeting basic needs - assisting in activities of daily living - assisting in the management of their mental health issues A psychiatrist is available as a part of this program.

With wraparound care at its centre, the Transitional Housing program empowers previously unhoused individuals to work towards long-term housing in the community in a safe, shared residence with 24/7 staff support.
