Special functions are: outreach, consumer identification, assessment, planning, direct service provision, intervention, and monitoring, evaluating, follow up, liaison, advocacy, consultation and collaboration. Provides ongoing support to help the individual function in the least restricted, most natural environment and achieve an improved quality of life. The case managers maintain involvement as the individual's needs change and cross service settings.

Housing subsidy for adults with a serious mental illness who are on limited income. Subsidies provided through lease agreements with landlords. Advocating between the individual and landlord to provide the housing and support for the individual.

Individuals supported in this program are those: who come into contact with the law, have the potential to come into contact with the law, and those who may have a developmental disability. These offenses must be considered low risk and the mental illness must be appropriately managed through community services.

This program has been designed to help individuals who have been identified as having an intellectual disability as well as: co-existing mental health disorders, long-endured trauma, addiction issues, as well as disrupted developmental and vocational experiences and involvement with the criminal justice system. A cohesive, interdisciplinary team of clinicians and support workers carry out these services including a psychiatrist specialized in Dual Diagnosis, a counsellor with expertise in trauma, addictions, and developmental disabilities, an occupational therapist, and a behavioural therapist. The team works with individuals to deliver intensive specialized psychiatric and behaviour supports to assist individuals to achieve and maintain a meaningful and satisfying life in their home communities.
