The program is designed to support persons with problematic substance use or concurrent mental health and substance use issues who are homeless, at risk of becoming homeless, or are considered inadequately housed.
Applicants must also demonstrate financial need. Participants can submit a written request to extend their time in the program after they have been in the program for six months. Extensions will be granted based on the individual’s identified recovery goals and their demonstrated active participation in the program. Services are delivered from an abstinence-based perspective.
Transitional housing is available to those who meet the above-noted criteria, who have recently participated in residential or day addictions treatment, or are planning to attend treatment, and are seeking to stabilize an abstinent recovery in the community.

The Peer2Peer Overdose Response program connects individuals affected by drug/alcohol poisoning with someone who has relevant lived experience. Our Peer Recovery Coaches are based in the community, RAAM clinics, and in the hospital to connect promptly with individuals with substance-related concerns, including drug/alcohol poisoning. The Peer Recovery Coach is also available to those experiencing harm related to substance abuse.
Peer Recovery Coaches at Emergency Rooms
Peer Recovery Coach provides support to individuals and their friends/families following a hospital presentation related to substance use, drug poisoning, and/or overdose. As part of the Peer Led Overdose Response program (P2P), the Peer Recovery Coach brings a unique, non-traditional perspective that offers practical and emotional support and a sense of hope, as individuals and families work through the first several days after an overdose and identify how they want to move forward.
The Peer Recovery Coaches are based at Grand River Hospital, Cambridge Memorial Hospital and Guelph General Hospital and respond when an individual is brought to the Emergency Department, providing care, support, education, and information in a timely manner. The Peer Recovery Coach will provide support to the individual and their family/friends while in hospital and briefly (for approximately 10 days) following discharge to support links to community resources and supports.

This service is designed for community residents whose substance use has resulted in criminal charges.
The Addiction Court Support Worker offers clients:
- brief motivational counseling to support positive changes regarding both substance use and criminal behaviour;
- assessment and referral to the most appropriate treatment and support resources;
- support to understand and comply with bail requirements

The Drug Treatment Court program is a specialized court process for non-violent people with addiction issues convicted of an offense. The program brings together judicial and social services who work towards positive change.

The Rapid Access Addiction Medicine Clinic (RAAM) is a specialized medical clinic designed for people who are experiencing health issues related to their drug and/or alcohol use.
The clinic is open to people of all ages who are currently using substances, and there is no cost to participate. RAAM is a short-term service and patients are connected back to primary care upon completion of their involvement.

The Rural Wellington Addictions service is delivered in collaboration with the Upper Grand Family Health Team.
The program provides clients with in-person assessments, treatment/care planning and counselling services. Individuals are also supported in accessing other community services based on their identified needs and/or goals. Education and support for friends/family of those who use substances is available. Services are provided by both an addictions counsellor and/or a peer support worker.
This service works together with the Rapid Access Addiction Medication Clinic (RAAM) and Community Withdrawal Support Service (CWSS) to provide addictions support to the residents of Wellington County.

The Addiction Support Coordination (ASC) program provides a range of services to members of specialized populations who also have addiction issues and to any service providers involved in their care.
These specialized populations include: those living with acquired brain injury; developmental delay/dual diagnosis (concurrent developmental delay and mental health issue); complex concurrent disorders; older adults/geriatric clients.
Services range from education and consultation for other service providers to direct support coordination work with clients. Support Coordinators are based in Cambridge, Guelph and Kitchener-Waterloo.

The Community Withdrawal Support Service (CWSS) offers an alternative to residential withdrawal management for individuals who can safely withdraw from alcohol and/or other drugs in a safe and supportive community environment.
Services include screening and assessing suitability for withdrawal in a community setting, and supporting people to prepare, plan for and complete community-based withdrawal. Services also include consultation and capacity building to referring organizations and primary care providers; education and support to family members/significant others; and referrals to other programs/services for treatment and relapse prevention.

The Aftercare program allows participants of the Stonehenge residential program to stay connected to the organization and staff for post-treatment support.
Aftercare is a regional program that is also open to people who have completed any residential addiction program in Ontario. Candidates can access the Aftercare support for up to two years after their treatment stay.

The Let's Grow Together program provides community treatment from a harm reduction perspective.
The program includes therapeutic groups; supported play with children; interactive education sessions re: substance use, parenting skills, infant-child mental health/development, nutrition, safety, obtaining employment, legal rights, etc.
Case management services are available to women who participate in the Let's Grow Together program. Support is provided on an as needed basis and is tailored to meet the needs of the individual. Services offered include brief one-on-one counselling; court support; advocacy; crisis management; community agency referrals and treatment planning.

The Rapid Access Addiction Medicine Clinic (RAAM) is a specialized medical clinic designed for people who are experiencing health issues related to their drug and/or alcohol use.
Individuals of all ages who are currently using substances are welcome. The RAAM is a short term service and participants are connected back to their primary care provider upon completion of their involvement.
This program is provided virtually to those residing in Waterloo and Wellington Counties.
