The Enhancing Care for Ontario Care Partners helps care partners across Ontario manage the stress and complexities of caring for persons living with dementia. The program provides both in-person and virtual clinical services and groups from 15 community health partner sites in over 40 local communities. The Reitman Centre for Alzheimer’s Support and Training at Mount Sinai Hospital is the provincial hub for the program. Care partners in Toronto can access the program at Mount Sinai through the Reitman Centre for Alzheimer’s Support and Training. Care partners outside the Toronto area can access clinical services and groups by using the referral information for their local area on https://carersontario.ca/ Caring for a family member with dementia is a challenging and emotionally demanding role. To help address some of the challenges faced by care partners, two groups are offered: 1. CARERS – (Coaching, Advocacy, Respite, Education, Relationship, Simulation) This is a skills-based group for care partners of a person living with dementia. Participants learn practical skills, problem-solving techniques, communication methods, and approaches to managing emotional stress which are tailored to the specific problems and needs of each care partner. 2. TEACH – (Training, Education and Assistance for Caregiving at Home) TEACH is an interactive group for care partners of a person living with dementia. Care partners may discuss themes such as self-care and future planning while learning from each other.

The clinic provides psychiatry consultation and time-limited treatment to adults 65 years or older who are experiencing issues with their mental health. Treatment is provided for illnesses such as dementia, anxiety, depression and bipolar disorder. Support for family members and other care partners providing care for patients is also provided.

Provides mental health assessment and treatment to Mount Sinai Hospital patients and their partners during pregnancy and the postpartum period for up to 1 year. Program is inclusive of all genders, sexual orientations and family structures. The Perinatal Mental Health Clinic provides mental health treatment for individuals experiencing symptoms of daily sadness or heightened worry, or for those who are having trouble functioning in daily activities, as well as those who have lost interest previously enjoyable activities. Care is provided those with a history of a psychiatric disorder in order to help prevent a relapse during pregnancy or postpartum and to help manage new challenges as they arise. Conditions include but are not limited to depression, anxiety, bipolar, Post-Traumatic Stress Disorder, eating and substance use disorders. The program is led by an inter-professional team of psychiatrists and mental health clinicians.

Provides assessment, diagnosis and treatment planning services for mental health-related illnesses for adults aged 18 to 64. The assessment process may include a screening phone interview with a mental health clinician, followed by an assessment which is generally done by a psychiatric resident and psychiatrist together. Program works with the individual to decide on a treatment care plan. The treatment care plan may include medication options, psycho-social interventions, individual or group therapies, or recommendations for educational tools and self-help resources. Depending on the care needs, the plan may include time limited follow-up visits in the clinic or direction to appropriate resources outside of Sinai Health. The plan will be communicated to the referring physician.

The HIV Psychiatry Clinic provides psychiatric consultation and time-limited treatment for adults over age 18 who are experiencing mental health-related illnesses and are living with HIV.

Serves adults experiencing severe, persistent, and complex mental health problems, including: those with marked impairment in social, occupational, and daily functioning; and those with special needs such as high demand for services, problematic substance use, homelessness, or involvement with the legal system. The program focuses on Chinese, Vietnamese, Korean and Tamil populations. ACTT offers an array of culturally and linguistically appropriate mental health services.

Provides culturally competent services for individuals of ethno-racial background who face mental health challenges and have been involved in the criminal justice system. The goal is to help individuals reduce or eliminate future recidivism and to improve their quality of life. Service includes psychiatric assessment and follow up, psycho-social and mental health education, consultation, direct brief service, court diversion, and case management support.

The Chinese Seniors Wellness Centre is designed to meet the mental health needs of the Chinese community by providing assessment and treatment in a culturally appropriate and sensitive environment. The Clinic provides treatment for Chinese speaking people who are 65 years old and above and for families and caregivers who face challenges in caring for an older person who has mental illness. Clinical Services include: psychogeriatric assessment, treatment – including pharmacotherapy and psychotherapy, psychoeducation, individual or group psychotherapy, counselling to caregivers and family members, shared care and referrals. In order to engage patients, the Wellness Centre also provides wellness programs include mental health education sessions, art for elderly virtual class, and more.
