Community Day Services (CDS) offers accessible supports, with a goal of providing 'as needed' and 'just in time' service to prevent the need for re-admission and to reinforce client-centered goals for recovery. CDS consists of three program components: - clinics - clozapine and injection clinics, dual diagnosis, metabolic monitoring - groups - individualized recovery supports - (1-3 sessions with a nurse, recreation therapist, dietician or spiritual care provider) Open to anyone who has a support in community.

Inpatient tertiary care for individuals with severe and persistent mental illness. Individualized shared care plans are developed based on client goals. A focus on strengths and hope is sustained to work with clients towards increased independence and reintegration to community. Creating a healing environment to promote health and enrich the lives of people living with mental illness and addictions. The team uses holistic approach including health and wellness, group and individual therapies.

This program offers client-centered intensive treatment and case management in the community, using a bio-psychosocial approach to rehabilitation. Referrals from applicants with severe and persistent mental illness (SPMI) / addictions are accepted. Intake committee processes referrals in line with need /service availability and according to provincial ACT standards.

Individuals are assessed at time of admission for medical/psychiatric stability and risk factors. Medical consultation provided through Grand River Hospital Emergency Department and individual's family doctors. Assessment tools administered following the crisis stage and prior to transfer to residential bed. Nurse practitioner on-site.

The Day Hospital provides short-term transitional support and treatment for adults who are experiencing intense mental health concerns. This is a day treatment service delivered through groups and individual support as needed.

The Children’s Eating Disorder Program provides out-patient care to youth with Anorexia Nervosa (atypical accepted), Bulimia Nervosa, and Avoidant Restrictive Food Intake Disorder. Family Based Treatment (FBT) is the offered intervention, with components of other treatment modalities added in to boost FBT effectiveness as needed. Youth are required to be followed by the eating disorder pediatricians

This service combines both therapeutic treatment and academic programming. The program offers milieu, individual and group therapy, family support/education, academic and physical education credits, life skills development, follow-up, consultation to significant supports in the community, system integration activities (Example: school, public health), and psychiatric assessment consultation if appropriate.

The Community Outreach and Treatment Team (COTT) provides treatment, rehabilitation and support through an intensive service coordination model to persons living with severe and persistent mental illnesses. Some services include outreach, injections and psychiatry. COTT also facilitates a support group for family and loved ones of individuals living with mental illness. Education, coordination, consultation and supports regarding Community Treatment Orders are also provided through COTT.

Hazelglen offers a home-based treatment alternative to inpatient care for individuals experiencing an acute psychiatric crisis. The team provides treatment and support (including 24-hour telephone support), utilizing a variety of interventions including psychopharmacology, family education and supportive therapy.

The child and adolescent mental health outpatient services provide multi-disciplinary, outpatient assessment, treatment, and consultation services. These services include: - rapid response service (urgent supports 1-3 visits) - transition support service - transitional medication follow-up service - psychiatric consultation (non-urgent consultations required by primary care)

This unit assesses, stabilizes and treats serious and acute psychiatric conditions including dual diagnosis for children. A Child Psychiatrist provides coverage to the Child and Adolescent In-Patient Program, 24 hours a day, seven days a week.

Model of service is assessment, diagnosis, and treatment recommendations for children between two and five years.

The Adult Outpatient Services provides multidisciplinary, outpatient assessment, treatment and consultation services. These services include: - Rapid Response Team (urgent support for one to three visits) - short-term psychiatric follow-up service (medication monitoring and transition to primary care) - psychiatric consultation (non-urgent consultation as requested by primary care)
