Community Withdrawal Support Program offers comprehensive assessment of needs; treatment planning; acute and post withdrawal assessment; case conferencing; interdisciplinary consultation; relapse prevention; community referrals; and education to individuals interested in withdrawal supports.

Transitional Case Management offers services to individuals struggling with their mental health needs which have resulted in an impairment to functioning in daily living skills, managing finances, housing, health, social recreation and employment domains.

Social, recreational and skill building opportunities are offered throughout the year across the region. Each group is an opportunity to meet new people, learn a new skill and enhance skills while connecting with staff who care about participants health and wellness. Most of the programs offered by Community Wellness are free.
Groups offered by the Community Wellness Teams fall into one of four categories:
- Social & Leisure: Groups and events to help build social skills and find meaningful activities to promote recovery
- Physical Wellness & Health: Activities, education and support to improve physical health
- Personal Awareness & Growth: Teaching life and living skills to support living with a mental illness
- Creative Self-Expression: Groups and learning opportunities to encourage creativity and self-expression.

Transitional Shared Living Services provides housing and support for youth who live with mental illness.

RAAM (Rapid Access to Addictions Medicine) provides Opioid Replacement Therapy (Suboxone only) to individuals who meet the criteria for Opioid Use Disorder. The program also provides medical treatment for Alcohol Use Disorder.

Community Homes for Opportunity (CHO) provide housing, meals, medication management, and support services in a home setting for people with serious mental illness who require 24 hour supportive care.

Counselling services for individuals who have historical or current use of opioids that may or may not be accessing substitution therapies. Community Education is also available as requested.

Crossing Bridges is a confidential, virtual safe space for young people to connect with an addiction counsellor, get information, and find services and supports for both substance use addiction and behavioural addiction (e.g. gambling, gaming, sex, Internet or social media use, etc.).

The Peer Support Transitional Discharge Model (TDM) program offers one-to-one services to individuals (Peers) who are looking for additional support following discharge from an inpatient mental health stay. One-to-one support is offered on a time limited basis.

The Peer Support - Youth program offers intentional, one-to-one services to youth who have an expressed need for support for mental health and/or addictions-related challenges. The Youth Peer Support Worker can assist the youth (peer) to identify their own wellness needs and support them to move towards an individual sense of wellbeing.
